•  Rome Memorial Hospital’s Primary Care Practices earn Recognition for Quality, Focus on Patient Care

    All of Rome Memorial Hospital’s primary care practices have earned recognition as a Patient Centered Medical Home (PCMH), an acknowledgement of the practices’ patient centered approach to care.
    Rome Medical Group, Delta Medical, Boonville Family Care and Camden Family Care have all earned recognition from the National Committee for Quality Assurance (NCQA), a national not-for-profit organization dedicated to promoting quality in health care. To earn this level-three achievement, each of these primary care practices completed a rigorous process aimed at making primary care more accessible, comprehensive and coordinated, while focusing on quality and safety to improve patient outcomes and lower overall healthcare costs.
    “Prompted by the need to improve the health of our patient population, enhance the outcome experience and be a recognized national partner in how care is delivered and paid for, we embarked on a journey to achieve PCMH recognition,” said Harisa Begic, PCMH project manager.
    “A Patient Centered Medical Home is best described as a primary care team that puts the needs of the patient first and every initiative begins with the benefit to the patient in mind,” Begic added.
    Patient Centered Medical Homes facilitate a partnership between you, your primary care provider and the specialists involved with your care to provide you with a continuum of care that best meets your needs.
    “A Patient Centered Medical Home (PCMH) designation means that each practice is committed to guiding patients through the often complex health care system,” said Rosemel Atkinson, vice president of physician practices at Rome Memorial Hospital. “Providers make treatment decisions with their patients, taking into consideration the patients’ preferences. They help their patients become engaged in their own healthy decisions and health care.”
    “Everyone in the practice – from our clinicians to front desk staff – works as a team to coordinate care, including specialists and community resources, if needed,” Atkinson continued. “With a medical home, you have a care team that supports and helps you make the best decisions for your health.”

    The PCMH model encompasses input from the American College of Physicians (ACP), the American Academy of Family Physicians (AAFP), the American Academy of Pediatrics (AAP) and the American Osteopathic Association (AOA).
